Add FAN number to suspension details

As a League we get numerous players picking up ground bans from coaching team Youth teams. We have players with the same name playing in the League. We have no way of identifying who is actually banned from playing by just having the name of the offender.

We either need FAN number or the name of the team/club the player is registered to in our League
