Addition of team name

We have observed that clubs are including their full club name against in teams' names when affiliating it.  This is causing complications during county cup (and possibly League) integration over to Full-Time.  Due to the limited number of characters Full-Time can use for a team name we are losing specific information to identify what team is involved in a match.

The simple solution appears to be to change the instruction on the Team details page to:

What is the team name? (this field can be left blank)
